• Complete all items on the form.  If a question is not applicable to you, mark it "N/A."
  
• Please type, or write strongly and clearly.
  
• The following attachments must accompany your application:
  
  
  • Legible copy of your IRS Determination Letter, showing your 501(c)3 status
  • Latest annual budget for your organization
  • A detailed project/program budget
  • Latest IRS Form 990, including Schedule A
  • List of your governing board and executive staff
  • Organization and project brochures, annual report, or other publications, as available
  
  
• Mail grant application and all attachments to the Bucks County Women's Fund,
  PO Box 1617, Doylestown, PA 18901, or deliver them to 956 Town Center Office Park,
  New Britain, PA, to be received in our New Britain office by 4 p.m. on Monday, November 16, 2009.
  No exceptions, please.
